Multi-threaded thread releases read selector prior to joining read thread

Description

This causes the read selector to be accessed after its release. Detected by valgrind.

Problem applies to POSIX, not Windows

fix incomplete apparently.

While debugging this a variety of other issues where found and repaired on recent modifications of the POSIX shared memory machinery.
